The below form authorizes regularly scheduled charges to your debit/credit card. You will be charged the amount agreed upon in your proposal and indicated below each billing period. You will also be notified of any extra payments through your monthly invoice which, if any, will be added to the amount charged from your account or card. A receipt for each payment will be provided to you.
